The Primary Care Network (PCN) of Northumberland is a collaborative network of family physicians, nurse practitioners, and primary care teams working together to improve access to care and strengthen primary care services across Northumberland County.
Working alongside the Ontario Health Team of Northumberland (OHT-N), the PCN supports clinical collaboration, provider engagement, education, quality improvement, and innovative initiatives that improve patient care and strengthen the local health system.
