SCOPE
SCOPE is one of several initiatives advancing the Northumberland Primary Care Network's commitment to improving access to comprehensive primary care. By connecting providers with specialist advice, community resources, and coordinated navigation support, SCOPE helps strengthen the patient medical home, reduce administrative burden, and improve timely access to care. As part of the broader Primary Care Access and Attachment Initiative, SCOPE complements other local programs, including Supported Attachment, Health Care Connect, and Interprofessional Care Team expansion.
SCOPE is a program designed to improve access to specialist advice, diagnostic services, and community resources. By providing timely support to primary care providers, SCOPE helps patients receive the right care at the right time while reducing unnecessary emergency department visits and specialist referrals.

Meet the SCOPE Nurse Navigator
The SCOPE Nurse Navigator works with the Supported Attachment Nurse Navigation team and serves as the central point of contact for participating providers assisting with complex care coordination, specialist navigation, referral support, and connecting patients to appropriate community resources.
