The Ontario Health Team of Northumberland (OHT-N) Primary Care Network (PCN) brings together primary care providers from across Northumberland County to connect, collaborate and strengthen primary care in our community.
By joining the PCN, you become part of a growing network of providers working together to address shared challenges, improve access to care and help shape the future of local primary care.
The PCN also creates opportunities for primary care providers to have a stronger voice within the Ontario Health Team of Northumberland. Your experience and perspective can help inform priorities, initiatives and decisions that affect both providers and the patients and communities they serve.
Who can join?
The OHT-N Primary Care Network is open to patient-facing primary care providers serving patients in Northumberland County.
This includes family physicians, nurse practitioners and other eligible primary care providers who want to connect with colleagues and contribute to strengthening primary care in our region.
