Supported Attachment

Helping Connect Patients to Primary Care Providers in Northumberland County

The OHT of Northumberland Primary Care Network is committed to ensuring every resident has access to a primary care provider. Supported Attachment is a collaborative program that helps connect unattached patients to participating family physicians and nurse practitioners while providing navigation and support throughout the attachment process. Working closely with primary care providers, Health Care Connect, and community partners, the program improves access to care, strengthens the patient medical home, and supports smoother transitions into primary care.

Pre-Attachment Support

  • Verify patient information

  • Confirm Health Care Connect status

  • Coordinate attachment opportunities

  • Prepare patients for joining a new practice

  • Connect patients with available community resources while they wait

Post-Attachment Support

  • Assist providers with onboarding newly attached patients

  • Promote continuity of care during the transition into the practice

  • Support completion of intake, required forms and referrals
