Primary Care Network of Northumberlands Relationship to the Ontario Health Team of Northumberland

The Primary Care Network is a key partner of the Ontario Health Team of Northumberland (OHT-N).

While the OHT-N brings together health system partners across sectors, the PCN provides a dedicated forum for primary care leadership, collaboration, and provider engagement.

Together, we work to improve access, coordination, and patient outcomes across Northumberland County.

Dr. Fraser Cameron

OHT-N Physician Lead, Chair, Primary Care Leadership Council

Originally from Toronto, Fraser completed his undergraduate degree in Biology at Queens University. He went to the University of Western Ontario for his medical school and completed his residency in family medicine in London.  Dr.  Fraser Cameron moved to Northumberland County in the Fall of 2015 opening his practice in Port Hope. Dr. Cameron lives locally in the area and maintains privileges at Northumberland Hills Hospital since 2015. He enjoys hospital medicine and takes a special interest in geriatric medicine, sports injuries and palliative care.

Stephanie Ross

Nurse Practitioner

A Nurse Practitioner who has a 25-year career across a variety of clinical, leadership and educational roles. Having grown up in Northumberland County and worked in the region for more than 20 years, she brings a strong understanding of the local health care system and a longstanding commitment to improving care for the communities she serves. She is particularly passionate about building a more integrated, equitable, and sustainable health care system—one that recognizes the important contributions of all health professionals and keeps patients and communities at the centre of decision-making. She looks forward to continuing to work collaboratively with health care partners, clinicians, and community leaders to help create meaningful and lasting improvements in health care.

Dr. Jennifer Braun

Family Physician

A family physician at the Community Health Centres of Northumberland.  She completed her undergraduate degree in Arts and Science as well as Medical school at McMaster University.  She completed her Family Medicine residency at the Memorial University of Newfoundland.  After graduation she spent some time doing locums around Newfoundland before returning to Ontario to work at the Guelph Community Health Centre.  After 10 years in Guelph she moved back closer to home and has been working in Port Hope at the CHCN for the past 8 years.

Dr. Steve Cornish

Family Physician

A family physician with a comprehensive practice in Port Hope. Dr. Cornish joined the Ganaraska Family Health Organization in 2020 and the medical professional staff of Northumberland Hills Hospital in 2021. Dr. Cornish trained at Western University, completing his undergraduate medical and Family Medicine residency training there between 2011-2017. He was a locum Family Physician for the London Family Health Team and held an inpatient locum position at St. Joseph's Healthcare in London before re-locating to Port Hope. Through his involvement with the Primary Care Network, Dr. Cornish hopes to promote the goal of equitable, comprehensive family medical care for all residents of the Northumberland region.

Theo Barker

Executive Director, Northumberland Family Health Team

Bringing more than 11 years of clinical and leadership experience to the organization. Beginning his career as a registered physiotherapist, Theo brings a strong frontline perspective to his collaborative, people-focused approach to leadership. Theo is passionate about primary care as the foundation of a strong and sustainable healthcare system and believes that meaningful relationships with staff, providers, partners, and the community are essential to delivering exceptional care. Proud to serve Northumberland County, Theo is committed to strengthening local primary care through collaboration, innovation, and community-focused solutions that support the health and well-being of residents across the region.

Dr. Michelle Cohen

Family Physician

A family physician in rural southeastern Ontario and an adjunct assistant professor in the Department of Family Medicine at Queen's University. She is a writer and researcher on health equity, health policy, and wellness pseudoscience. She is a contributing author to 2024's Breaking Canadians: Healthcare, Advocacy and the Toll of COVID-19. Her upcoming book Her Body, Her Cure: On the Personal & Political in Wellness Culture is coming out in early 2027.

Nancy Hill

Nurse Practitioner

A nurse practitioner (NP) with Northumberland Family Health Team (NFHT). Nancy graduated from Queen's University and has 33 years of emergency/ICU prior becoming a NP. Nancy looks forward to holistically looking after her patients and their loved one through the ages.
